    This online Graduating Issues form is for FSS students only. If you are not an FSS student, seek assistance from your Faculty. This also applies if querying FSS Minors.

    This form is for Graduating Students ONLY. 

    Please note that Minor(s) not on Pass list requests will only be entertained once the Minor(s) was/were previously declared.
